| How Does the Ordering Process Work? |
| First have a proper measurement of your
patient's foot taken. We recommend using the Brannock Device. Include
length and width. Decide on the best model shoe for your patient.
You may order it 3 different ways: online, by phone ( 1-877-405- FOOT)
or by fax ( 1-888-405-FOOT) |

| How long does it take to fulfill
an order? |
| Your shoes and Pre-Fab inserts will be
shipped out within 2 business days if they are in stock. If not in
stock you will be notified. All shoes not in stock will be ordered
from the manufacturer. This will add another 10 business days to the
order. Shoes ordered with Custom Inserts will take about 2 weeks to
be shipped out. |

| Can orders be sent to different
locations for multioffice practices? |
| Orders can be sent to different office
locations as long as the offices are under the same Tax ID number.

| What is our shipping policy? |
| We send all our shoes and inserts via ground
UPS and is included in our package price. Our package price also includes
free shipping for exchanges and free shipping of your foam boxes to
our warehouse. There is additional charges for 2nd day air, Hawaii
and Alaska shipments. |

| What is our returned items policy? |
| Straight returns can be made within 30
days of purchase. All shoes must come in the original box and be unworn.
Inserts must come in the original package and not be worn or molded.
Shipping is at your own expense. Same policy for exchanges of shoes
except we will send you a return label on request for the return.
Custom Inserts are not returnable. |

| How does Medicare reimburse for
Diabetic Shoes? |
| Medicare reimburses $59 per shoe or $118
per pair. You must use the code A5500 left or right to get reimbursed.
Medicare pays for 1 pair of Diabetic shoes per year. |

| How does Medicare reimburse for
Custom Moldable Insoles? |
| Medicare pays $48.67 per pair of Pre-Fab
Diabetic inserts. They allow 3 pairs per year for a total amount of
$198. You must bill A5512 right and left to get reimbursed. Medicare
pays $72 for one pair of Custom Inserts or $216 for 3 pairs. You must
bill A5513 right and left to get reimbursed. |

provider? |
